Payment is due at the time of treatment. To enable you to proceed without delay our office offers several financial options. We encourage you to select financial arrangements that work best in your budget. For your convienence we offer the following options:
Cash , Personal check, Visa, Master Card, Discover & American Express, Care Credit
We will be more than happy to file your dental insurance information for you. Please understand that the benefit information obtained from your benefit provider is an estimate, not a guarantee of payment. Any fees not covered by your insurance company will be solely your responsibility.
Appliances will not be seated permanently until the account is paid in full.
Occupational Safety and Hazard Administration (OSHA) mandates all of the sterilization guidelines followed in this office. For this reason we request that only patients being seen by the doctor or the hygienist be allowed in the operatories at all times. This is clarified in stating that no parents will come back with children and no children will come back with parent.
Children under the age of ten may not be left unattended in the reception area at any time. Our patient coordinator, due to the attention required to do her job, does not have the ability to keep her full attention on the reception area.
Due to OSHA standards, No food or drink are allowed in the reception area or operatories at any time.
Our office strives to provide preventive dental medicine. We take a full mouth set of x-rays on all new patients followed by minimal necessary films once a year for diagnostic purposes. This process aides us in detecting problems as early as possible. Due to the great benefit that diagnostic x-rays provide our office, any patient refusing x-rays will be refused treatment.
We understand that your time is valuable. We strive to remain on schedule, but being a Doctor's office emergencies do occur. In an effort to be courteous to our patients, if we are running behind we will try to contact you prior to your appointment. In the same effort to be courteous, we request that patients running more than ten minutes late call and let us know.
If you are later than fifteen minutes for an appointment it will be necessary to reschedule to avoid complications with other patients. There will be a fee assessed for cancellations or no shows.
If you are unable to keep an appointment, we ask that you kindly provide us with a minimum two business day notice. Our office does NOT accept cancellations or changes in appointments after hours, by voice mail, you must call during business hours. This courtesy on your part will make it possible to give your appointment to another patient who needs to see the dentist or hygienist.
MISSED APPOINTMENT FEE OF $50.00 WILL BE CHARGED TO ANY PATIENT WHO DOES NOT NOTIFY OUR OFFICE WITHIN 2 BUSINESS DAYS TO CANCEL OR RESCHEDULE THEIR APPOINTMENT.
INSURANCE POLICY: Please be prepared to show your insurance card and driver's license at the time of your visit. If the patient has any insurance changes , it is the patients responsibility to provide new information. If this information is not provided at the time of service, patient will then be responsible for charges incurred.
Please understand that your dental insurance is a contract between the insurance carrier and yourself, not between Dental Arts of Frisco and the insurance carrier. Our office will gladly submit your insurance claim for you as a courtesy. The patient is responsible for their co-payment at the time of service. Please be aware that not all insurance covers all procedures performed in our office. If our office has not received payment with in 45 days, the patient is responsible for these charges.
